Doch als die Brüder Luigi und Mario den Schaden beheben wollen, erleben sie ihr blaues Wunder: Zwei völlig abgedrehte Typen packen Daisy und ziehen sie mit in die Tiefe des Tunnels. Luigi und Mario nehmen sofort die Verfolgung auf und landen so in Dinohattan, einer aggressiven, lebensfeindlichen Metropole, deren Bewohner sich in beängstigendem Maße als Fleischfresser entpuppen. Regiert wird diese Unterwelt von dem dämonischen Echsenkönig Koopa, der Daisy wegen eines in ihrem Besitz befindlichen magischen Steins kidnappen ließ. Die beiden Brüder stürmen heldenhaft die Festung von Koopa und befreien Daisy. Doch dazu müssen sie vorher gegen Koopas hirnlose Polizeitruppe, die Goombas, verschiedene monströse Mutanten und verrückte Menschenfresser kämpfen.
